Information you provide
If you use the contact form, SafeFare may receive your name, telephone number, email address, topic, message, and campaign or page source.
If you request a Medicare Starting-Point Report, SafeFare may receive your first name, email address, ZIP code, and the questionnaire answers you choose to provide. The report questionnaire does not ask for a phone number.
Please do not submit Social Security numbers, Medicare numbers, banking information, insurance member IDs, medical records, or detailed diagnoses through the website.
How the information is used
Information submitted through the regular contact form is used to respond to your request, communicate about insurance options, maintain business records, improve the website, and meet legal or regulatory obligations.
Information submitted through the Medicare Starting-Point Report pilot is used to prepare and deliver the report the visitor requested, maintain the minimum records needed to operate and evaluate the pilot, and meet applicable legal or regulatory obligations. A report request by itself is not treated as a request for a sales call or as enrollment in a marketing email list.
Contact form provider
The website uses Web3Forms to process and forward form submissions to SafeFare. That provider may process technical and submission data under its own terms and privacy practices. You can avoid the form by calling or emailing Roderick directly.
Calls, texts, and email
If you submit the regular contact form, the consent language shown on that form governs SafeFare's response to that request, including contact by email, mail, telephone, or text as stated there.
If you submit the Medicare Starting-Point Report questionnaire, SafeFare uses the supplied email address to deliver the requested report. That report request alone does not authorize SafeFare to call or text you and does not add you to a marketing email list. If you later contact SafeFare or separately ask for help, that later request may create a new communication relationship.
Website and hosting data
The website and its hosting providers may collect standard technical information such as IP address, browser type, device type, requested pages, dates, and diagnostic logs for security, reliability, and performance. SafeFare may use aggregate analytics if they are added to the site.
Sharing and sale
SafeFare does not sell personal information. Information may be shared with service providers that operate the website or communications, with insurers or administrators when you ask SafeFare to pursue coverage, or when disclosure is required by law. Insurance applications have additional privacy notices and authorizations.
Retention and security
SafeFare keeps information only as long as reasonably needed for the purposes described, business records, and legal obligations, subject to service-provider practices. No internet transmission or storage system can be guaranteed completely secure.
